The Opportunity

A new Medicare payment model. A significant infrastructure gap.

CMS launched ACCESS, Advancing Chronic Care with Effective, Scalable Solutions, to fund technology supported care for Medicare beneficiaries managing chronic conditions. The program creates outcome aligned payments for four clinical tracks: cardio kidney metabolic, musculoskeletal, behavioral health, and early stage cardiometabolic risk.

The opportunity is real and the funding is already allocated. The barrier is operational. Participating organizations must manage CMS enrollment, maintain compliant reporting pipelines, contract with qualifying technology partners, and coordinate care delivery across facilities, simultaneously. For most nursing facilities and provider groups, that infrastructure does not exist.

CMS Compliance and Reporting

Outcome tracking, quarterly and annual CMS reporting, audit-ready data pipelines, and performance documentation across all enrolled patients and clinical tracks.

Facility Contracting

RethinkX.ai contracts directly with nursing facility partners, manages revenue distribution, and handles the operational relationship so facilities can participate without building their own ACCESS program.

Digital Health Partner Integration

We connect vetted digital health technology partners, covering each ACCESS clinical track, to the enrolled patient population. Partners provide the care technology. We provide the distribution and compliance infrastructure.

The Model

How RethinkX.ai and its partners work together.

RethinkX.ai operates as the ACCESS participation and orchestration layer. We receive CMS outcome aligned payments, distribute revenue to facility partners and technology partners, and manage the compliance infrastructure that makes the whole model function.

Facility partners provide patient access and care delivery context. RethinkX.ai handles CMS enrollment, compliance, and reporting on their behalf. Facilities receive a revenue share from each enrolled patient without building their own ACCESS infrastructure.

Digital health technology partners provide the care platforms, remote monitoring, behavioral coaching, condition-specific interventions, across each ACCESS clinical track. RethinkX.ai provides the enrolled patient population and handles the CMS reporting requirements.
